Job description

Nurse Practitioner (NP) – Nephrology Practice

Location - San Mateo, CA

Statue - Part Time Wednesday and Friday 9a-5p

Overview

This is an excellent opportunity for a Nurse Practitioner to join a collaborative nephrology practice focused on delivering high-quality, patient-centered kidney care. The NP will work closely with nephrologists and interdisciplinary teams to manage patients with Chronic Kidney Disease (CKD), emphasizing education, care coordination, and comprehensive disease management.


Key Responsibilities

Clinical Care & Patient Management

  • Deliver holistic ESKD care management, including:
  • Patient activation and engagement
  • Depression screening and support
  • High-risk hospitalization oversight
  • Transitions of care
  • Management of co-morbid medical conditions
  • Conduct annual comprehensive ESKD disease management visits
  • Provide CKD clinical care and kidney disease education

Care Coordination & Collaboration

  • Collaborate with the nephrologist
  • Participate in care planning and communication across the continuum of care to support optimal patient outcomes

Clinical Functions

  • Perform physical examinations and assess patient health status
  • Order, interpret, and evaluate diagnostic tests and laboratory studies
  • Develop, implement, and adjust treatment plans as needed
  • Prescribe medications in accordance with scope of practice and state regulations
  • Document patient encounters accurately within the electronic medical record
  • Educate and counsel patients and families on diagnoses, treatment plans, preventative care, and healthy lifestyle behaviors

Professional Practice

  • Promote a high standard of patient service and professionalism
  • Build effective working relationships across clinical and operational teams
  • Participate in team-based care and adhere to organizational policies and procedures
  • Maintain regular, reliable attendance

Qualifications

Required

  • Current Nurse Practitioner (Advanced Practice) license in the state of practice
  • Appropriate specialty certification
  • Will train new grad
  • Strong understanding of physician practice operations, protocols, and workflows
  • Excellent written and verbal communication skills

Preferred

  • Prior nephrology and/or dialysis experience
