Job description

Transition Coach

Purpose
Transition Coaches provide direct care and therapeutic support to adolescents in a residential treatment setting. Through relationship-based, trauma-informed care, they help students build emotional regulation, resilience, and independence while preparing for life beyond treatment.

Schedule:

  • Monday, Wednesday, Friday 7am - 3pm
  • Monday, Wednesday, Friday 2:30pm - 3pm
  • Tuesday, Thursday, Saturday 7am - 3pm
  • Tuesday, Thursday, Saturday 2:30pm - 3pm
  • Each schedule will require one full Sunday shift per month, 8am - 10pm. 

Key Responsibilities

  • Build strong, professional relationships with students through daily engagement and activities
  • Support emotional regulation, accountability, and real-world skill development
  • Implement treatment plans and provide behavioral support, including crisis intervention when needed
  • Maintain a safe, structured, and supportive environment
  • Supervise students during routines, recreation, and off-campus activities
  • Transport students to appointments and outings (as needed)
  • Complete documentation and communicate with the treatment team
  • Contribute to a positive, team-oriented work environment

Qualifications

  • Experience working with youth (residential, education, recreation, or similar) preferred
  • Strong communication skills and ability to build trust while maintaining boundaries
  • Calm, dependable, and able to handle high-stress situations
  • Willing to be trained in CPR/First Aid and crisis intervention (provided)
  • Must be 20+ years old; valid driver's license preferred

Physical & Work Environment

  • Active role including standing, walking, hiking, and participating in recreational activities
  • Ability to respond to crises, including physical interventions when trained
  • Work occurs in residential homes, classrooms, and outdoor settings in varying conditions
