Job description

Job Title: Family / Internal / Geriatric Medicine Physician

Location: Minneapolis, MN

Shift: Full-time & Part-time position

Summary:

We are looking for a Physician to serve patients in Long-Term Care (LTC) and Transitional Care Units (TCUs).

Responsibilities:

  • Conduct comprehensive patient assessments and ongoing medical evaluations.
  • Create and manage personalized treatment plans based on patient health needs.
  • Diagnose and treat acute and chronic conditions, providing timely medical interventions.
  • Prescribe medications, order diagnostic tests, and modify treatments as needed.
  • Work closely with nurses, therapists, and social workers to coordinate comprehensive care.

Requirements:

  • MD or DO degree from an accredited medical school.
  • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred.
  • Valid medical license to practice in Minnesota.
  • Previous experience in long-term care, transitional care, or geriatrics is a plus.
  • Excellent problem-solving, communication, and teamwork skills.
