Job description

The Home Hospital Registered Nurse is responsible for providing the essential elements of the inpatient care delivery model into the patient's defined living space using leveraged technology and remote clinical care coordination. The Home Hospital RN provides quality care to patients through the implementation of the nursing process applied through a team nursing model. The Home Hospital RN will access and document the care of patients and will work collaboratively with a multi-disciplinary healthcare team to deliver therapeutic interventions within the patient's home while maintaining high quality and safety standards.

Essential Job functions:

Performs and documents nursing process - assessment, diagnosis, planning, implementation, and evaluation of patients receiving care remotely or in-person at their defined home. Reviews and follows-up on patient education, implemented patient care plans, and coordinated services. Triages, identifies, and acts on changes in patient condition.

Ensures quality care during daily in-person visits to patients receiving acute care at home through Home Hospital. This includes the ability to identify patient's needs through acute care and in-home assessments then implementing and monitoring the medical treatment plan in collaboration with the 24/7 virtual nursing team and providers.

Ensures quality patient care across the continuum, reduces avoidable readmissions, and improves outcomes for patients enrolled in the Home Hospital program.

Provides quality care and timely response during urgent in-person assessments when requested by the Home Hospital virtual nursing and provider team. Diagnostics and treatments within the scope of nursing may be requested by the team and communication back to the team regarding the assessment, results of diagnostics, and response to treatment will be crucial.

Provides and/or helps coordinate appropriate hospital resources for acute inpatient individuals who are receiving Home Hospital services during their stay including but not limited to therapy, food, consultative, mobile imaging, labs, transportation, medication administration, and medical equipment.

Provides high level nursing care through the ability to perform phone triage and intervene or escalate appropriately using critical thinking skills.

Utilizes and instruct patient and family on the use of technology resources used for biometric data capture, patient symptom reporting, and telehealth visits.

Ensures appropriate screening of patients in need of continuum integration services prior to hospital discharge including but not limited to transitional remote patient monitoring and telehealth virtual visits.

Provides direct bedside patient education and consultation on continuum integration services as needed.

Provides community resources appropriately to prevent un-necessary hospital admissions by collaborating with the interdisciplinary care team such as social work, virtual nursing team, care coordination assistants, and/or providers.

Ensures responsibility and visible commitment of diversity equity and inclusion strategy, programs, and initiatives through collaboration and implementation of initiatives across the health system.

Demonstrates personal ownership and accountability for safety across the area of responsibility by role modeling daily key concepts of a high reliability organization as personal work habits.

Performs other duties as assigned and/or participates in special projects to support the mission of VCUHS and the team. Accepts alternate responsibilities as required.

Patient Population:

Demonstrates the knowledge and skills necessary to provide equitable care appropriate to the age of the patients served on their assigned unit.

Demonstrates knowledge and related competencies of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to their age, specific needs and to provide the equitable care needed as described in departmental policies and procedures.

Employment Qualifications:
Required Education:
Diploma or Associates Degree from a professional nursing program recognized and/or approved by the corresponding state's Board of Nursing.

All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an approved RN to BSN program within two years of their start date and to complete the program within five years of their start date unless grandfathered under previous requirements.

Preferred Education:

Completion of a Bachelor's Degree in Nursing (or higher) from an accredited program

Licensure/Certification Required:

Current RN licensure in Virginia or eligible or compact state

AHA BLS HCP Certification or equivalent

Licensure/Certification Preferred:

Nursing Specialty certification

Minimum Qualifications:
Required Qualifications:

Minimum of three (3) years of critical care, acute care or medical/surgical RN experience, or equivalent clinical experience (as determined by VCUHS) in critical care, acute or medical/surgical with at least one (1) year of RN experience.

Other Knowledge, Skills and Abilities Required:

Follows documented physician/licensed independent provider orders.

Practices within the boundaries of the regulations governing the practice of nursing in the Commonwealth of Virginia.

Practice is guided by the ANA Code of Ethics for Nursing and established national nursing practice standards.
