  • Coordinate patient admissions and interfacility transfers
  • Review emergency department admissions for medical necessity and level of care
  • Collaborate with physicians, hospitals, payors, and care teams to ensure timely patient placement
  • Obtain authorizations and support regulatory compliance (EMTALA/COBRA)
  • Help optimize patient flow, continuity of care, and patient outcomes
  • Qualifications

    Required
  • Active California RN license (or willingness to obtain)
  • Minimum of two years of acute care case management hospital nursing experience
  • Strong clinical judgment, communication, and multitasking skills
