• Conduct detailed assessments of patients’ emotional, social, and environmental needs to develop individualized treatment plans that promote their well-being.

• Implement evidence-based therapeutic interventions tailored to the unique experiences and needs of patients, focusing on alleviating anxiety, depression, and trauma.

• Maintain precise and confidential clinical records that comply with regulatory standards, ensuring the highest level of patient confidentiality and care.

• Work collaboratively with a multidisciplinary team, including physicians, nurses, and other social workers, to coordinate comprehensive, patient-centered care.

• Educate patients and their families about available resources and coping strategies, providing support as they navigate the emotional landscape of hospice care.

The patients you will serve include those in hospice care, who may be facing terminal illness, significant life changes, or end-of-life decisions. • A Master's degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW or LCSW-R) license in New York State.

• Minimum of 2 years of clinical experience in a mental health setting, with a preference for experience in hospice or palliative care.

• Proficiency in solution-based psychotherapy techniques and trauma-informed care methodologies.
