Job Summary 
The Nurse Practitioner provides advanced clinical evaluation, diagnosis, and management of patients through telehealth platforms in support of Total Care Connectโ€™s Mobile Integrated Healthcare (MIH) programs. 

This role delivers real-time clinical oversight for Community Paramedics, conducts virtual post-discharge assessments, manages acute and chronic conditions remotely. The NP ensures high-quality, evidence-based care while maintaining regulatory compliance and value-based performance standards. 
 

Core Responsibilities 

Virtual Clinical Assessment & Management 

  • Conduct comprehensive telehealth evaluations. 

  • Diagnose and manage acute and chronic conditions. 

  • Provide treatment plans, medication management, and follow-up recommendations. 

  • Order diagnostics and laboratory testing as appropriate. 

  • Ensure medical necessity documentation aligns with payer requirements. 

Field Clinical Support (Community Paramedicine Integration) 

  • Provide real-time telehealth consultation to Community Paramedics during in-home visits. 

  • Escalate care appropriately when higher acuity is identified. 

  • Collaborate on care plan adjustments. 

  • Support protocol-driven interventions and standing orders. 

Clinical Documentation & Compliance 

  • Maintain timely, complete, and defensible EMR documentation. 

  • Ensure documentation supports billing and medical necessity. 

  • Adhere to licensure, telehealth regulations, CMS guidelines, and HIPAA standards. 

  • Participate in internal chart audits. 

Quality & Performance Participation 

  • Contribute to Quality Improvement initiatives. 

  • Support reduction in 30-day readmissions and ED utilization. 

  • Participate in case reviews. 

  • Support development of evidence-based care pathways. 

Interdisciplinary Collaboration 

  • Collaborate with Community Paramedics, Clinical Services Supervisor, Quality Coordinator, Operations Coordinator, and Care Coordination teams. 

  • Participate in interdisciplinary conferences. 

  • Communicate clinical updates to referring providers. 
     

Qualifications  

Required  

  • Licensed Nurse Practitioner (Ohio) with an active, unrestricted license. 

  • National board certification (FNP, AGNP, or similar). 

  • Prescriptive authority in Ohio. 

  • 3โ€“5 years of clinical experience. 

  • Experience with telehealth delivery. 

  • Proficiency with EMR systems. 

Preferred 

  • Experience in Mobile Integrated Healthcare or home-based care. 

  • Experience with Medicare Advantage populations. 

  • Experience in value-based care models. 

  • Acute care or emergency medicine background. 

  • DEA registration.
