Job description

Summary:Deliversprimary health care services to patients and provides individual and/or group healtheducation andcounseling while providing outreach to and advocacy for the homeless population.

EssentialDuties:

  1. Delivers primary health care services to patients in the clinic and in the mobile healthcare unit.

  1. Provides COVID services as needed intheclinic and in the mobile healthcare unit.

  1. Provides individual and/or group health education and counseling while providing outreach to and advocacy for the homeless population.

  1. Conducts physical examinationsofpatients. Exercises sound nursingjudgmentwheninitiating and prescribing therapy within the guidelines of written protocols.

  1. Interviews patients, documents their medical history and makes assessments and/or physical diagnosis of condition.

  1. Maintains proper clinical records and reports, using problem-oriented medical records which can be audited for accountability.

  1. Participates in program Quality Assurance (QA) committee and quality improvement efforts.

  1. Performs and completes PCMH duties asassigned.Duties include but are not limitedto:attending patient huddle,timelyclinical advice, involved in QI activities, and access needs.

  1. Initiates medications and treatmentwithin the scope of practice.

  1. Supervisesassignedclinical staff.

  1. Referspatients for specialized medical care beyond the scope ofthe HCHServices, including arrangement for any necessary ambulance transportation.

  1. Collaborates with theClinicalDirector in review/revision of policies,protocols,andproceduresas necessary.

  1. Responsible for covering after-hour calls for FHWC.

  1. Participates in and/or conducts in-service presentations.

  1. Performsall other duties as assigned.

Education, Experience &Training:

Must belicensedas a Registered Nurse Practitioner in the State of Kentucky; successful completion of a Nurse Practitioner training program in an accredited educational institution; and at least two years ofbroad-basedprofessional nursingexperience.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.Position requires occasional or frequent moderate physical activity.Whileperforming the duties of this job, the employeeis regularly required totalk orhear.The employeeis occasionally required tostand; walk, climb, balance, stoop, kneel, crouch, crawl, and use hands tomanipulate,handleor feel; andreach withhands and arms.The employeeis occasionally required tolift/and or exert force up to 50 pounds.

Other Requirements:

Must have a valid driver's license and the flexibility and willingness to change schedules and duties as needed according to staff availability.Must be willing totravelthe service area as needed.Travel is primarily local or within the service areaduring the business day, although some out-of-area and overnight travel may be expected.Agency policyregardingdrug and alcohol testing and criminal background check applies to this position.Mustmaintaina high levelof professionalism,demonstratedby a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency.
