About the Facility – Mobile Medcare (fixed location) MobileMed serves as a primary care medical home for thousands of low-income adults in Montgomery County, Maryland. Each week, MobileMed has 6 primary care clinics and 2 specialty care clinics. Under normal operating schedule there is a combination of fixed and mobile medical van sites. However due to COVID-19, currently patients are only being seen at fixed site locations such as this one. Within their primary care program, services are provided in English, Spanish, French, Korean, Amharic, Vietnamese, and Farsi. Facility Address: 1500 E Gude Dr, Rockville, MD 20850

Responsibilities
  • Must possess high-level assessment skills that are needed to determine patient's well-being and current status. Patients presenting at a clinic or by phone may require intervention by the RN to determine their need for immediate care. RN needs to be comfortable with physical assessment skills and the ability to assess patients for serious illness, including those with limited English proficiency.
  • Follows up with Specialty Referral Coordinator, Catholic Charities and others to ensure timely and accurate specialty referrals.
  • Acts as a liaison with local hospital staff, including emergency, laboratory, and radiology departments, as related to the provision of care in assigned clinic(s).
  • Follows up on patient visits and ensure patient compliance with referrals and diagnostic testing; acts on all critical or abnormal tests as appropriate.
  • Must recognize that the complexity of the patient population are often unfamiliar with navigating the healthcare system and that extra attention needs to be placed on making sure patients understand the process.
  • Works with Transition Care Coordinator to ensure appropriate information Is present for the patient's visit and to provide the Coordinator with information for reporting purposes.
  • Supervises in day-to-day operations, which includes discharge planning, medication distribution, appointment set-up, application processing, co-pay collection, depositing money, and patient education.
  • Addresses any issues concerning roles and responsibilities among clinic staff.
  • Promptly brings any personnel or clinic issues to the attention of the Clinical Director. Works with Clinical Director on staff hiring and discipline as well as professional development.
  • Actively participates in Quality Improvement initiatives, engaging other clinic staff.
  • Understands and ensures compliance with the company's Policies and Procedures.
  • Is responsible for the clinic flow. Approves walk-ins and makes assignment changes as necessary to improve patient access and volume when possible. Makes clinic appointments when necessary.
  • Oversees data (e.g. lab results) into the eClinical Works (eCW) electronic health record system.
  • Reviews provider notes, including orders, codes, and follow up appointments. Locks volunteer providers' notes after review.
  • Communicates with volunteer providers regarding scheduling, documentation, referrals and medications.
  • Ensures appropriate ordering and inventory of medical/office supplies and medications. Communicates with centralized administrative staff regarding ordering as well as any scheduling concerns.
  • Responsible for safe handling and scheduling pick-up of biohazardous waste in assigned clinic(s), and for compliance with applicable OHSA standards.
  • Ensures that broken equipment is isolated and identified and notifies the Assistant Clinical Director by email.
  • Attends and participates in All-Staff Meetings and Nurse/Provider Meetings as scheduled. Participates in team activities as directed.
Qualifications
  • Registered nurse in the state of Maryland.
  • Ability to interact with multicultural population and providers.
  • Excellent organizational skills. Management experience strongly preferred.
  • Electronic health record experience strongly preferred.
  • Experience in not-for-profit healthcare organization a plus.
  • Current CPR certification for healthcare workers.
  • Fluency in English required; additional languages, especially Spanish, preferred.
