Home Health Registered Nurse (RN) – Full-Time
DuPage & Cook Counties, IL + surrounding areas
Salary: Up to $120,000

Nightingale Home Healthcare is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care and case management to patients in the home setting. This position is ideal for nurses who value independence, flexible scheduling, and meaningful one-on-one patient care.

Home health experience is required.

Position Overview

The Registered Nurse (RN) provides direct patient care and manages an assigned caseload in the home setting in accordance with physician orders and agency policies. Responsibilities include patient admissions, comprehensive assessments, care planning, and coordination of services.

Key Responsibilities
  • Conduct patient admissions and OASIS assessments

  • Obtain medical histories and perform physical assessments including vital signs, mobility, and mental status

  • Develop and implement individualized plans of care

  • Provide skilled nursing services while maintaining HIPAA compliance

  • Evaluate patient progress and adjust care plans as needed

  • Coordinate care with physicians and interdisciplinary healthcare teams

  • Supervise LPNs, Home Health Aides, and Personal Care Attendants

  • Submit timely and accurate clinical documentation

  • Coordinate referrals for additional services such as physical therapy and social services

Qualifications
  • Active Registered Nurse (RN) license

  • Home Health experience REQUIRED

  • OASIS and WellSky (Kinnser) experience preferred

  • Med-Surg and wound care experience preferred

  • Current CPR certification

  • Current TB test and physical (within the last 12 months)

  • Valid driver’s license, reliable transportation, and proof of auto insurance

  • Ability to attend 1–3 day orientation at our Willowbrook office
