Job description

University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant who will deliver care for patients on the University of Minnesota Heart Care team at M Health Fairview Heart Care Clinics in Maplewood, MNand Woodbury, MN, as well as at St. John's Hospital in Maplewood.

This position will provide patients with expert patient care and coordination of services. The role will assist the practice to improve patient satisfaction and enhance the quality of care provided to patients through increased access and coordinated services.


In This Role, You Will:

  • Join a multidisciplinary team of 20 Cardiologists, 1 Heart Failure focused, and 15 Advanced Practice Providers.
  • Coordinate and provide care for adults with cardiac diseases throughout the course of evaluation and treatment.
  • Provide outpatient management for adults with heart failure of various etiologies, with potential forrounding and inpatient management of heart failure patients
  • Provide patient and family education as it relates to heart failure and general cardiology.
  • Provide clinic coverage in heart failure and general cardiology.
  • Perform clinic follow up visits and potentially helps with inpatient consults.
  • Show commitment to exemplary patient care.
  • Manage patient care through appropriate use of available resources.
  • Demonstrate ability to deliver care or service adjusting approaches to reflect developmental level of population served.
  • Utilize a fully integrated Electronic Medical Record (EPIC).

Schedule:

  • 1.0 FTE
  • Monday-Friday, daytime hours
  • No weekends, no holidays


We're Looking For a PA or NP Who Is:

  • A graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
  • Eligible or is certified through NCCPA, ANCC or AANP.
  • Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN APRN license or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP's).
  • Able to meet threshold criteria/qualifications for credentialing and privileges.
