Stamford/Norwalk| Full-Time, Part-Time, or Per Diem Opportunities Available


Compensation:
This is a paid per visit position with routine visit rates starting at $35.00 per visit!

Position Overview

The LPN is responsible for providing direct patient care and case management to an assigned caseload in accordance with physician orders. You’ll help patients heal safely at home by delivering skilled, compassionate care and supporting the care team in developing individualized treatment plans.


Key Responsibilities

• Obtain medical history and assess patient conditions
• Conduct physical assessments including vitals and mental status
• Provide skilled nursing care per physician’s plan of care
• Maintain patient confidentiality in compliance with HIPAA
• Evaluate patient progress and communicate updates to the team
• Submit all visit notes and documentation promptly
• Collaborate with RNs and other care team members
• Educate patients and families on care plans and home management

Qualifications

• Active LPN license in CT
• Physical exam within the past 12 months
• Current TB test (within 12 months) and CPR certification
• Valid driver’s license 
• Ability to attend a 2-day orientation at our Stamford office
