Job description

We are seeking experienced and motivated Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for patients throughout Eastern MO. This is an excellent opportunity for providers looking to supplement their income with a flexible, part-time schedule.

Key Responsibilities:

  • Conduct comprehensive in-home Health Risk Assessments (HRAs)
  • Identify and document acute and chronic medical conditions
  • Obtain vital signs and calculate BMI
  • Review medications and pharmacological therapy
  • Perform a physical examination
  • Evaluate patients’ physical, mental, and social health needs
  • Educate and counsel patients and their families on health conditions and preventative care

Schedule & Flexibility:

  • Part-time, flexible scheduling
  • Set your own hours
  • Visits available 7 days a week
  • Minimum commitment of 8–10 hours per week

Compensation:

  • $90 per completed in-home assessment
  • Mileage reimbursement provided
  • No-show compensation included

Qualifications:

  • Active Nurse Practitioner license in the state of Missouri
  • Experience with Health Risk Assessments (HRA) preferred
  • Strong clinical assessment and documentation skills
  • Ability to work independently and manage schedule efficiently
  • Reliable transportation for in-home visits
