Job description

Job Summary:

The Rural Health Program Coordinator provides oversight and coordination of Remote Patient Monitoring (RPM) services to ensure program effectiveness, compliance, and successful achievement of organizational and grant-related objectives. This position is responsible for program analytics, reporting, policy development, budget monitoring, and coordination with internal and external stakeholders. The Rural Health Coordinator serves as the primary liaison between Midtown Health Center, funding agencies, and contractors while advocating for the best interests of Midtown Health Center and its patients.


Supervisory Responsibilities:

None

Duties/Responsibilities:

  1. Provides oversight and coordination of RPM services and related program activities.
  2. Analyzes program data and maintains reporting metrics related to program outcomes, utilization, demographics, and evaluation measures.
  3. Assists with pre-award activities including grant applications, work plan development, and supporting documentation.
  4. Develops, implements, and maintains program policies and procedures to ensure compliance and operational efficiency.
  5. Monitors RPM work plans, timelines, deliverables, and budget compliance to ensure program goals are achieved.
  6. Maintains accurate and organized program records and documentation.
  7. Prepares and submits all required reports related to program activities, accomplishments, challenges, statistics, evaluations, and demographic data in a timely manner.
  8. Ensures timely submission of reports and required documentation to funding sources and regulatory agencies.
  9. Serves as the primary contact and liaison with grantors, contractors, vendors, and partner representatives on behalf of Midtown.
  10. Represents Midtown in a professional manner while advocating for the interests of the organization and its patients to establish mutually beneficial expectations and outcomes.
  11. Collaborates with leadership and interdisciplinary teams to support program implementation and continuous improvement initiatives.
  12. Identifies opportunities for workflow improvement and operational efficiencies within RPM services.
  13. Performs other duties as assigned.

Duties 1 through 12 are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.

  • Ability to efficiently carry out Midtown Healthโ€™s Center mission statement and comply with its policies and procedures.
  • Ability to establish and maintain effective working relationships with patients, co-workers, and the public and to be able to demonstrate cultural awareness and sensitivity.
  • Must be able to maintain strict confidentiality of sensitive information in accordance to HIPAA regulations.
  • Ability to comply with all occupational safety and health standards and all rules, regulations, and orders issued under the Occupational Safety and Health Act (OSHA).
  • Ability to utilize critical thinking and problem solving skills to address clinicโ€™s needs.
  • Ability to interpret oral and written instructions and do appropriate follow-up.
  • Ability to work independently and as a team member.
  • Ability to navigate electronic health record (EHR) systems and communicable disease reporting systems.
  • Ability to engage individuals and/or groups, including experience with public speaking and presentations.
  • Must have exceptional time management and organizational skills.
  • Must possess excellent interpersonal and communication skills in one-on-one, group, and telephone interactions.
  • Ability to constructively participate in quality improvement efforts.
  • Ability to actively participate in marketing and outreach activities when requested and emergency response events when required.
  • Bilingual (English/Spanish) verbal and written skills highly desired.
  • Knowledge of community resources and the ability to assess the need for referrals.


Education and Experience, Licensure and Certification:

  • Nursing or bachelorโ€™s degree in health education, healthcare administration, public health, or related field preferred.
  • Minimum of two years of program management experience in a community health or healthcare setting preferred.
  • Experience with grant management, reporting, healthcare analytics, or Remote Patient Monitoring services preferred.
  • Must have a valid driverโ€™s license and the ability to utilize own transportation for work purposes.
