Job description

The Community Action Coalition for South Central Wisconsin is a tri-county non-profit organization serving the counties of Dane, Jefferson and Waukesha that works in a coordinated and comprehensive manner to develop and implement community-driven programs and services that will make a critical difference in the lives of low-income families and individuals, that will advance the revitalization of low-income communities, and that will ultimately provide empowerment of low-income families and individuals in rural and urban areas.  The Case Manager I provides direct support to individuals and families seeking assistance through Community Action Coalition programs. This role focuses on intake, assessment, service planning, and connecting participants with community resources. Case Manager I is an entry to mid-level position designed for individuals who are developing their professional expertise in client services.
