Responsible for direct supervision to patients, indirect supervision of rehabilitation technicians. General supervision of PT assistants in accordance with Oklahama Practice Act. As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans while partnering with patients, caregivers, physicians, and interdisciplinary team members to deliver exceptional care. This role operates under the supervision of Therapy Leadership and in accordance with state practice acts, departmental standards, and Mercy's mission, values, and service standards.

What You'll DoPatient Evaluation & Care Planning
  • Perform comprehensive physical therapy evaluations and establish individualized plans of care.
  • Assess patient needs using objective testing and clinical judgment.
  • Collaborate with patients and caregivers to identify functional goals and expected outcomes.
  • Develop age-appropriate treatment plans for adult and geriatric populations.
  • Communicate treatment strategies, goals, and expectations clearly and professionally.
  • Coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team.
  • Complete accurate and timely documentation of evaluations, plans of care, and required assessment tools.
Treatment & Rehabilitation
  • Deliver evidence-based physical therapy interventions, therapeutic exercises, and treatment modalities.
  • Monitor progress and modify treatment plans based on patient response and clinical outcomes.
  • Educate patients and caregivers on exercises, safety precautions, and home programs.
  • Ensure safe treatment practices and compliance with infection control standards.
  • Maintain productivity expectations while delivering high-quality patient care.
  • Communicate patient progress and significant changes to physicians and care teams.
Documentation & Compliance
  • Maintain accurate, timely, and compliant clinical documentation.
  • Complete daily progress notes, interim evaluations, discharge summaries, and patient education records.
  • Track and document patient outcomes, goals, and functional progress.
  • Ensure all patient charges and documentation are completed according to departmental standards.
Professional Development & Team Collaboration
  • Participate in departmental meetings, hospital initiatives, quality improvement projects, and continuing education.
  • Maintain required licensure, certifications, and competencies.
  • Assist with orientation and training of new staff and students as needed.
  • Support a culture of learning, collaboration, and excellence across the rehabilitation department.
QualificationsRequired Education
  • Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an accredited program.
Required Licensure & Certifications
  • Current or temporary Physical Therapist license in the state of practice.
  • Current BLS (Basic Life Support) certification.
  • All required certifications must remain current before independently caring for patients.
Preferred Experience
  • Six months to one year of physical therapy experience preferred.
  • Experience working with adult and geriatric populations in a rehabilitation setting preferred.
Physical Requirements
  • Regularly lift, push, and pull up to 50 pounds.
  • Stand and walk for extended periods throughout the shift.
  • Frequently bend, kneel, squat, twist, reach, and perform hands-on patient care activities.
