Job description

Find your calling at Mercy! The Rural Health Transformation Program Coordinator provides operational leadership for a Rural Health Transformation Program hub and is responsible for implementation, governance support, program performance, and supervision of Navigator staff serving participating counties. The Coordinator serves as the primary liaison between Mission leadership, community stakeholders, Hub Boards, and the Regional Coordinating Network. Position Details: Minimum Qualifications
  • Bachelor’s degree required in Public Health, Social Work, Healthcare Administration, Community Health, or related field.
  • Three to five years of experience in community health, program coordination, healthcare outreach, or related field.
  • Demonstrated leadership, supervisory, or team coordination experience.
  • Strong communication, facilitation, organizational, and project management skills.
Preferred Qualifications
  • Master’s degree preferred.
  • Experience managing multi-site or community-based programs.
  • Familiarity with rural health initiatives, government-funded programs, and community partnerships.
  • Completion of a Community Health Worker training program (such as Partners in Care) preferred at hire.
  • Ability to obtain Community Health Worker credentialing preferred.
  • Grant writing, grant administration, or grant reporting experience preferred.
