Our Assisted Living Memory Care unit is designed to provide a secure, engaging, and supportive environment tailored specifically to residents living with Alzheimer’s, dementia, and other memory challenges.

The Role

We are seeking a compassionate, skilled Licensed Practical Nurse (LPN) to join our Memory Care nursing team.

In this role, you will be the clinical heart of our Memory Care unit. You will deliver personalized nursing care, administer medications and treatments, guide direct care staff, and support residents and their families through every stage of their memory care journey. Key Responsibilities
  • Specialized Memory Care: Deliver individualized clinical care tailored to residents with cognitive impairment, maintaining a calm, structured, and compassionate routine.

  • Medication & Treatment Administration: Safely administer medications, treatments, and perform skin care/wound documentation in accordance with physician orders and Indiana state regulations.

  • Resident Monitoring & Assessment: Observe resident condition changes, conduct routine health assessments, and promptly report changes to the Charge Nurse/Director of Nursing and family members.

  • Team Guidance: Provide clinical direction, mentorship, and support to Resident Care Assistants (RCAs) / CNAs on the Memory Care floor.

  • Family Communication & Care Planning: Collaborate with interdisciplinary care teams and maintain empathetic, clear communication with families regarding their loved one's care plan and progress.

Qualifications
  • Licensure: Active, unencumbered license as a Licensed Practical Nurse (LPN) in the State of Indiana (or compact state equivalent).

  • Certifications: Current CPR / BLS certification required.

  • Experience: Prior LPN experience in Assisted Living, Memory Care, or Long-Term Care is highly preferred. Experience working with Alzheimer’s or dementia care is a huge plus!

  • Skills: Exceptional patience, strong clinical assessment abilities, crisis de-escalation techniques, and a warm, empathetic communication style.
