Registered Nurse -Lake County Territory
Compassionate Care Hospice (CCH), an Amedisys Company
**This position is based in Clermont, FL and requires travel to assigned area(s) in Lake County Schedule: Flex - min. 10hrs./wk.

Hourly Range:
$38 - $42/ hr. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training
You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.
Responsibilities
  • Performs initial nursing assessment on new admissions.
  • Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
  • Regularly communicates patient progress to the clinical manager, physician, and care team.
  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Educates families on patient condition, needs and care.
  • Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
  • Performs other duties as assigned.

Qualifications
Required:
  • Current RN license in the state(s) where you will work
  • Minimum 1 year of clinical RN experience in an acute healthcare setting
  • Current CPR certification

Preferred:
  • Hospice or home health experience
  • Strong communication and interpersonal skills
  • Ability to work independently and manage multiple priorities
  • Comfortable with daily travel between facilities
  • Proficient in electronic medical record (EMR) systems
