As a Hospice Admissions RN You will: 

  • Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care 
  • Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents 
  • Obtain medical history, current medications, and symptoms 
  • Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines 
  • Coordinate the start of services (nursing, aides, equipment, medications.) 
  • Collaborate with interdisciplinary team 
  • Maintain accurate and timely documentation
