As a Registered Nurse, you'll contribute to our success in the following ways:

  • Conduct start-of-care visits, supervisory visits, and client assessments to ensure quality care delivery

  • Develop, review, and revise individualized plans of care based on patient needs

  • Supervise homemakers, personal care aides, and home health aides to ensure compliance and quality standards are met

  • Monitor and report significant changes in patients' physical and mental conditions

  • Educate caregivers, patients, and families using effective teaching and learning principles

  • Ensure all services meet State, Federal, contract, and licensure requirements

  • Conduct caregiver orientations and performance assessments as required

  • Evaluate home environments for safety risks and recommend solutions to improve patient safety

  • Maintain accurate clinical documentation, care plans, and service records

  • Communicate professionally with caregivers, clients, case managers, and leadership teams

  • Participate in staff meetings, in-services, and continuing education opportunities to support professional growth

What is Required?

  • Graduate of an accredited School of Nursing

  • Current Registered Nurse (RN) license in good standing in the state applying for

  • CPR Certification required

  • Minimum of one (1) year of nursing experience; Home Care experience preferred

  • Knowledge of Medicare/Medicaid, home care benefits, and applicable State regulations preferred

  • Strong clinical assessment, communication, and organizational skills

  • Excellent computer proficiency and documentation abilities

  • Ability to multitask and adapt in a fast-paced healthcare environment

  • Professional verbal and written communication skills

  • Ability to motivate and lead caregivers in a positive and supportive manner

  • Reliable personal transportation is required

  • Ability to travel locally, and regionally to support additional field offices as needed

You will report to the Director of Nursing or Clinical Manager.
