Activa Home Health, a licensed, Medicare-certified home health agency, is seeking experienced and compassionate Field Registered Nurses to provide Medicare home health visits on a per diem basis.

Position Summary

The Field RN provides skilled nursing care for Medicare home health patients in accordance with each patient's individualized Plan of Care, with a focus on safety, outcomes, and patient satisfaction.

Experience with WellSky Kinnser, OASIS, and Medicare home health is required.

Key Responsibilities

  • Deliver skilled nursing care during Medicare home health visits in accordance with the Plan of Care
  • Accurately complete and document visits in the EMR, including OASIS assessments
  • Communicate promptly with the interdisciplinary care team regarding any changes in patient condition
  • Provide clinical services within scope of practice, including:

    • Medication administration
    • Wound care
    • Infusion therapy
    • Catheter care
    • Safe transferring and positioning of patients


  • Collaborate with the Nurse Supervisor and Manager of Clinical Practice
  • Demonstrate professionalism, empathy, attention to detail, and strong clinical judgment

Required Qualifications

  • Active Florida Registered Nurse license
  • Medicare home health experience required
  • OASIS experience required
  • Wound care experience required
  • Proficiency with electronic medical record systems (WellSky Kinnser preferred)
  • Minimum of 1 year of home health experience
  • Strong communication, teamwork, and customer service skills
