Role Overview:
The Home Health RN provides skilled nursing services to patients in their homes, supporting recovery and long-term health management. This position requires independent clinical judgment, strong organizational skills, and a focus on delivering patient-centered care.

Key Responsibilities:

  • Assess patient health status during home visits and identify care needs

  • Develop and carry out individualized care plans in accordance with physician orders

  • Provide nursing care such as medication administration, wound care, and monitoring of chronic conditions

  • Track patient progress and adjust care as necessary

  • Maintain clear, accurate, and timely clinical documentation

  • Educate patients and caregivers on treatment plans, medications, and home care techniques

  • Communicate effectively with physicians and other healthcare providers

  • Promote safety and infection control within the home environment

Qualifications:

  • Active Registered Nurse license

  • Graduate of an accredited nursing program

  • Current CPR/BLS certification

  • Previous clinical experience required; home health experience preferred
