Position Overview

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing services to patients in their homes throughout Shepherdsville, Kentucky, including Bullitt and Jefferson Counties. This role is responsible for completing start of care admissions, conducting comprehensive assessments, managing patient care plans, and coordinating with the interdisciplinary team to ensure high-quality, compliant patient care. The ideal candidate is clinically skilled, organized, and committed to delivering exceptional patient-centered care in the home health setting.

Schedule & Work Details

  • Location: Shepherdsville, KY, including Bullitt and Jefferson Counties
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 32 hours
  • Pay Rate: $51.00 - $53.00 per hour
  • On-Call Requirement: One weekend per month
  • Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities

  • Provide skilled nursing care to patients in their homes in accordance with physician orders and agency policies.
  • Complete start of care (SOC) admissions, comprehensive assessments, and ongoing patient evaluations.
  • Develop, implement, and update individualized plans of care based on patient needs and physician directives.
  • Monitor patient progress and identify changes in condition, communicating updates to the appropriate members of the care team.
  • Coordinate care with physicians, therapists, home health aides, and other interdisciplinary team members.
  • Ensure accurate, thorough, and timely clinical documentation in accordance with agency policies and regulatory requirements.
  • Educate patients and caregivers on disease management, medications, treatments, safety practices, and self-care techniques.
  • Maintain compliance with Medicare, state, federal, and agency guidelines.
  • Participate in case conferences and interdisciplinary team meetings as required.
  • Support quality initiatives focused on improving patient outcomes and satisfaction.

Required Qualifications

  • Active Registered Nurse (RN) license in the state of Kentucky.
  • Current driver's license, reliable transportation, and proof of automobile insurance.
  • Strong clinical assessment and patient care skills.
  • Ability to work independently and effectively manage a field-based caseload.
  • Excellent communication, critical thinking, and organizational skills.
  • Proficiency with electronic health record (EHR) documentation systems.
  • Preferred Qualifications
  • Previous home health nursing experience.
  • Experience completing Start of Care (SOC) visits and comprehensive patient assessments.
  • Knowledge of OASIS documentation and Medicare home health regulations.
