About the Role

As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes, following physician orders and Medicare guidelines. Youโ€™ll work independently in the field while being fully supported by our Clinical Managers, QA team, and office staff.

Key Responsibilities
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits

  • Complete comprehensive assessments and develop individualized plans of care

  • Provide skilled nursing services including medication management, wound care, patient education, and chronic disease management

  • Accurately complete OASIS and clinical documentation in our EMR in a timely manner

  • Collaborate with physicians, therapists, and interdisciplinary team members

  • Educate patients and caregivers to promote safety, independence, and improved outcomes

  • Adhere to all Medicare, ACHC, and agency policies and procedures

Qualifications
  • Active RN license in the state of Florida (required)

  • 1 year of Medicare home health experience strongly preferred

  • Experience with Wellsky/Kinnser preferred

  • OASIS experience preferred

  • Current CPR certification

  • Valid driverโ€™s license, reliable transportation, and active auto insurance

  • Strong clinical assessment and time-management skills

  • Comfortable working independently while staying connected to a team
