We are currently seeking compassionate and experienced PRN Registered Nurses (RNs) to join our team. Position Summary

The PRN Registered Nurse is responsible for providing skilled nursing care to patients in their homes, completing comprehensive assessments, developing individualized care plans, and coordinating care with physicians and the interdisciplinary team. This position offers flexibility while allowing you to build lasting relationships with your patients.

Responsibilities
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Evaluation, and Routine Skilled Nursing visits.
  • Complete comprehensive patient assessments and develop individualized plans of care.
  • Administer medications, wound care, injections, IV therapy, catheter care, and other skilled nursing interventions as ordered.
  • Monitor patient progress and communicate changes in condition to the physician and care team.
  • Educate patients and caregivers on disease management, medications, home safety, and treatment plans.
  • Maintain accurate, timely documentation in the electronic medical record.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure quality patient care.
  • Ensure compliance with Medicare Conditions of Participation, agency policies, and state regulations.
  • Current, active Florida Registered Nurse (RN) license.
  • 1+ years of nursing experience required.
  • Home Health and Medicare experience preferred.
  • OASIS experience preferred.
  • Current BLS/CPR Certification.
  • Valid Florida driver's license, reliable transportation, and current auto insurance.
  • Excellent assessment, communication, and organizational skills.
  • Ability to work independently and manage a flexible schedule.
  • Experience with WellSky/Kinnser is a plus.
