Job description

Full‑time Registered Nurse (RN) – Home Health – Arlington Heights, IL

Responsibilities
  • Provide skilled nursing visits and comprehensive patient assessments in the home.
  • Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
  • Complete accurate and timely OASIS assessments and clinical documentation.
  • Educate patients, families, and caregivers on disease management, medications, treatments, and home safety.
  • Monitor patient progress and communicate changes in condition to physicians and other healthcare providers.
  • Coordinate care with physical therapists, occupational therapists, speech therapists, home health aides, and other members of the care team.
  • Ensure compliance with CMS, ACHC, Medicare, and agency policies and procedures.
  • Participate in case conferences and quality improvement initiatives.
  • Maintain accurate documentation using the agency's electronic medical record (EMR) system.
Qualifications
  • Active Illinois Registered Nurse (RN) license.
  • Minimum of 2 years of home health nursing experience preferred.
  • OASIS experience preferred.
  • Strong knowledge of Medicare home health regulations.
  • Experience with Axxess EMR preferred.
  • Excellent assessment, organizational, and communication skills.
  • Ability to work independently while managing multiple patient visits.
  • Current CPR certification required.
