Position Overview
The Registered Nurse (RN) plays a critical role in delivering skilled nursing services to patients in their homes. This role involves comprehensive patient assessment, care planning, OASIS documentation, coordination of services, and supervision of field staff as applicable.
This is an ideal role for a nurse who values autonomy, clinical excellence, and meaningful patient relationships.
Key Responsibilities
  • Conduct comprehensive skilled nursing visits including admissions, recertifications, resumptions of care, and discharges
  • Complete accurate and timely OASIS documentation
  • Develop, implement, and update individualized Plans of Care in collaboration with physicians
  • Assess patient conditions, monitor progress, and identify changes requiring intervention
  • Educate patients and caregivers on diagnoses, medications, treatments, and home care techniques
  • Coordinate care with therapists, case managers, physicians, and support staff
  • Supervise and provide clinical oversight to LPNs and HHAs as assigned
  • Ensure compliance with Medicare, Medicaid, and agency policies
  • Participate in case conferences, quality improvement initiatives, and required trainings

Qualifications
  • Active Missouri Registered Nurse (RN) license
  • Minimum of 1 year RN experience (home health experience preferred)
