Job Title: Registered Nurse (RN) โ€“ Home Health Care
Job Location: Oakland, MI
Job Type: Contract

Job Description:
  • Conduct patient assessments and develop individualized care plans
  • Administer medications, treatments, and skilled nursing procedures
  • Provide wound care, IV therapy, catheter care, and patient education
  • Monitor patient progress and communicate changes to physicians
  • Coordinate care with therapists, social workers, aides, and other healthcare professionals
  • Supervise LPNs and Home Health Aides as required
  • Complete accurate and timely documentation in compliance with Medicare, CMS, and agency standards
  • Participate in case conferences and quality improvement activities

Requirement:

  • Active RN license in good standing
  • Minimum 1 year of clinical nursing experience (Home Health preferred)
  • OASIS experience preferred
  • Current BLS certification
  • Valid driver's license and reliable transportation
  • Strong assessment, documentation, and communication skills
