Job description

Occupational Therapist

The Occupational Therapist provides care to clients in their home accordance with the plan of care and with participation by the family, nurse or other persons as necessary. All functions shall be performed in accordance with the established policies and practices of the agency.

Servicing Areas: Middletown | Millsboro | Dover | Milford | Seaford

Essential Duties:

  • Guide and instruct the client in prescribing therapeutic self-care and create activities that are directed toward improving independence and physical and cognitive functioning
  • Participate in the development of the plan of care
  • Prepare a diagnostic evaluation report of each client and make recommendations
  • Prepare clinical and progress notes
  • Advise, consult and teach appropriate treatment procedures and supportive activities to clients, family member and other agency staff
  • Establish household management routines
  • Design and/or order appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning
  • Instruct clients and their families in activities of daily living
  • Attend care conferences as deemed necessary
  • Instruct the family in the client's total occupational therapy program
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, and HIPAA regulations and requirements
  • Abides by all regulations, policies, procedures and standards
  • Performs other duties as assigned

Position Requirements & Competencies:

  • Graduate of an accredited occupational therapy curriculum
  • Certified as an occupational therapist by AOTA, American Occupational Therapy Association
  • Current Occupational Therapist license in the state in which practicing
  • Minimum one year of work experience as an Occupational Therapist
  • Must have a valid Driver's license
  • Must possess current CPR and First Aid Certification
  • Must demonstrate ability to exercise independent judgment with limited supervision
  • Must be comfortable working in a home health care setting within a client's residence
  • Excellent verbal and written communication skills
  • Flexible schedule, including availability to work evenings, weekends and holidays as needed
  • Must be well organized and, excellent to time-management and attention to details skill
