Registered Nurse โ€“ย Aluma Home Care

Position Summary: The Registered Nurse (RN) in a non-medical home care setting is responsible for providing high-quality care to clients in their homes. This role includes conducting initial intake assessments, developing and updating individualized care plans, performing follow-up assessments, and supervising personal care attendants.

Key Responsibilities:

  • Conducts Home Health Aide Orientation
    • Reviews and certifies caregiver skills competency checklist.
    • Infection Control review and certification.
    • Reviews Care Plan with each aide for knowledge and understanding.
  • Initial Intake Assessment:
    • Conduct comprehensive assessments to evaluate clientsโ€™ health status and care needs.
    • Collect medical histories, current medications, and other relevant information.
  • Care Plan Development:
    • Develop individualized care plans based on assessment findings, client preferences, and best practices.
    • Collaborate with clients and families to set achievable goals and desired outcomes.
  • Care Plan Updates:
    • Regularly review and update care plans to reflect any changes in clientsโ€™ conditions or needs.
    • Document all modifications accurately and promptly.
  • Follow-Up Assessments:
