ESSENTIAL DUTIES AND RESPONSIBILITIES (included but not limited to): We are looking for candidates who want to use their expertise to improve health, wellness and care access for patients with chronic conditions.

We are recruiting candidates that experience with cardiology patients, able to develop purposeful relationships with patients, and can provide clear reporting to the cardiology care team.
