Schedule:

Monday to Friday: 3PM to 6PM | Saturday: 10AM to 3PM

Summary: 

Advanced Care Partners, one of Inc. 5000's fastest-growing companies, is seeking a kind and dedicated Certified Nurse Assistant/Home Health Aide(HHA) to provide essential care for medically fragile children and adults in their homes or schools.     • Base: $16 per hour 

    • Weekend Differential (7pm Friday through 7am Monday): $1.50 per hour 

    • Night Differential (7pm-7am): $1.00 per hour 

    • Holiday Differential: $1.50 per hour

Responsibilities: 

  • Assist our patients with daily care such as grooming, bathing, and toileting.

  • Assist with medication; take and record vitals.

  • Assess home situations to determine that our patients are in the best possible environment.

  • Provide emotional support to our patients and their families.

  • Provide First Aid and CPR when necessary.

  • Act as a home health educator for our patients and their families.

Required Qualifications: 

  • Active CPR certification required (ONLINE COMPANIES ARE NOT ACCEPTABLE).

  • Certified Nurse Assistant required(CNA)/ preferred(HHA)

  • Valid PPD skin test
