Reintegration Case Manager (CM) / Community-Based Rehabilitative Specialist (CBRS)Location: Boise, Idaho and surrounding communities
Position Type: Full-Time or Part-Time
Schedule: Monday–Friday, primarily 9:00 AM–5:00 PM
Work Environment: Boise, Idaho and surrounding communities. Work is completed in the field, with access to office locations as needed.

One Love Agency is seeking compassionate, motivated, and relationship-focused Case Managers (CM) and Community-Based Rehabilitative Specialists (CBRS) to support adults transitioning from incarceration back into their communities.
Our Reintegration Program serves justice-involved individuals who are working to rebuild their lives through stable housing, employment, behavioral health treatment, recovery supports, healthy relationships, and community connection. We help clients overcome barriers, develop life skills, access resources, and build a foundation for long-term success.

Responsibilities
  • Provide Case Management and/or CBRS services to adults with mental health, behavioral health, and reintegration needs.
  • Complete assessments and assist clients in identifying strengths, barriers, goals, and support needs.
  • Develop individualized service plans in collaboration with clients and their support systems.
  • Assist clients with accessing housing, employment opportunities, educational resources, healthcare, transportation, and community services.
  • Teach and reinforce independent living skills, coping skills, problem-solving skills, communication skills, and community integration strategies.
  • Support clients in reducing barriers that contribute to instability and recidivism.
  • Advocate for clients and assist them in navigating complex systems and community resources.
  • Collaborate with probation and parole officers, treatment providers, employers, housing agencies, and other community partners.
  • Monitor client progress and adjust interventions as needed to support successful outcomes.
  • Maintain accurate, timely, and compliant documentation in accordance with Medicaid and agency standards.
Qualifications
  • Bachelor's degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or a related field preferred.
  • Relevant experience may be considered in lieu of degree requirements where applicable.
  • Experience working with mental health, substance use, justice-involved populations, or community-based services preferred.
  • Strong communication, organization, and problem-solving skills.
  • Ability to build rapport while maintaining professional boundaries.
  • Ability to work independently and collaboratively within a team environment.
  • Valid driver's license, reliable transportation, and ability to travel throughout the service area.
  • Experience with electronic documentation systems is a plus.
Preferred Experience
  • Working with justice-involved or formerly incarcerated individuals.
  • Probation, parole, corrections, or community supervision systems.
  • Behavioral health and substance use treatment services.
  • Housing and employment resource coordination.
  • Case management or rehabilitation services.

Compensation
