MDS Coordinator

Job Summary:

We are seeking a highly motivated and organized MDS Coordinator to join our team at Ventura Services Florida in Miami, FL. As a key member of our healthcare team, you will play a critical role in ensuring the accurate and timely completion of Minimum Data Sets (MDS) for our residents. Responsibilities:

  • Coordinate the collection and submission of accurate and complete MDS data in accordance with federal regulations and company policies

  • Develop and maintain relationships with residents, families, and healthcare team members to ensure a comprehensive understanding of resident needs and preferences

  • Conduct and document detailed assessments of resident needs and preferences, including social, emotional, and environmental factors

  • Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that promote resident well-being and quality of life

  • Monitor and report on MDS data to identify trends and areas for improvement, and make recommendations for changes to the care plan

  • Participate in quality improvement initiatives and contribute to the development of policies and procedures related to MDS coordination

  • Maintain accurate and up-to-date records and Files, including MDS assessments, care plans, and other relevant documentation
