Overview
We are seeking an experienced RN MDS Coordinator to support assessment, care planning, and clinical operations in a skilled nursing setting. This is a full-time role with a balanced mix of MDS coordination and hands-on clinical support.
Schedule Structure
  • Approx. 24 hrs/week: MDS Coordination
  • Approx. 16 hrs/week: Resident Care Manager (RCM) support
  • Additional floor coverage as needed (call-ins, staffing support)

Key Responsibilities
  • Coordinate MDS, CAAs, and care plans
  • Set ARDs and manage MDS scheduling
  • Ensure accurate documentation and regulatory compliance
  • Collaborate with IDT and support Medicare processes
