MDS Coordinator – RN or LPN | Luxor Healthcare | Full-Time

Are you an experienced MDS coordinator looking to join a dedicated skilled nursing facility backed by an experienced corporate team that's genuinely invested in your success? We're looking for an experienced MDS Coordinator (RN or LPN) who wants to do their best work in a place where they'll actually be heard.

If you're skilled in the RAI process, know Medicare/Medicaid regulations inside and out, and want a role where your work directly shapes resident outcomes, this is worth a closer look.

What You'll Do:

  • Own the full RAI/MDS process from assessment through care plan completion, in compliance with CMS regulations
  • Partner with the Interdisciplinary Team to build individualized, meaningful care plans
  • Conduct or coordinate resident interviews and clinical assessments
  • Identify needs for specialized evaluations (behavioral, psychotropic, skin integrity, etc.)
  • Ensure timely, accurate documentation and certification of all assessments
  • Contribute data and analysis to support our Quality Assurance Program

What We're Looking For:

  • Active RN or LPN license in Missouri
  • Prior MDS Coordinator experience (this is a must)
  • Strong working knowledge of Medicare/Medicaid billing and compliance
