Job description

HealthPlus Staffing is seeking a Nurse Case Manager in Chicago, IL. The Nurse Case Manager will provide help to high risk infants and children getting health care and developmental services.
 
 
Nurse Case Manager will be responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from High Risk Infant follow Up Program from the State of Illinois.
Job Requirements:
  • Bachelor Degree in Nursing from an Accredited College or University
  • Minimum of 2 years' experience working in Community Health or Maternal Child and Health nursing
  • Must have Active nurse License in the state of Illinois
  • Compensation: Commensurate on experience
  • Benefits: Full Benefits 
  • Pediatrics Schedule: Monday to Friday
  • Location: Chicago, IL
  • Reliably commute or planning to relocate before starting work (Required)
  • Education: Bachelor's (Preferred)
  • Experience: Nursing: 1 year (Preferred)
