Job description

Job Purpose: To facilitate the improved wellbeing of individuals with severe mental illness and/or substance use disorders. The Case Manager will work collaboratively with an integrated treatment team to provide clinical direction and support to a pool of clients. Case managers will develop and implement a treatment plan, which will include evolving goals and client-centered interventions.

Essential Job Functions:

Builds therapeutic rapport with clients to aid in identifying treatment needs to develop client-centered treatment plans.
Provides psychosocial rehabilitation, symptom monitoring & management, and supportive counseling to clients in the CSP program.
Actively participates in daily team meetings, adhering to a team approach to best aid all clients assigned to the team.
Monitors clients' psychiatric medication regime and compliance as directed by the physician.
Provides crisis intervention and supportive counseling to clients who are in crisis.
Maintains a treatment-oriented environment (under supervision of program supervisor) and assures consistency in carrying out treatment objectives.
Coordinates utilization of various community resources, day programs, and outside health care providers.
Assists clients in financial planning and the development of income resources and coordinate with representative payee program as needed.
Provides treatment-related education and liaison with the client's family and natural supports.
Assists clients in securing adequate housing and prompting and training skills necessary to keep housing.
Provision of ongoing assessment of clients demonstrated in case note submission, biannual assessments, maintaining case files as required by agency policy, and applicable local, state and federal codes.
Meets department-designated monthly billing quotas, EHR note entry within 24 hours, biannual treatment plans and other funder required documentation.
Formulates appropriate discharge plan with the assistance of client and program manager.
Attends and participates in staff meetings, in-service training seminars, and conferences as required.
Adheres to agency and Milwaukee County (funder) policies and procedures while carrying out daily responsibilities.
Collaborates and assists with all levels of personnel within the program to achieve common goals.
Provides client transportation in personal vehicle as needed.
Other duties as assigned.

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)

Minimum Required Education: Master's degree in behavioral science or related field

Minimum Required Experience: 1000 hours of post-degree clinical experience with individuals with severe and persistent mental illness

Required Auto Insurance: Vehicle liability insurance in accordance with Agency policy


Knowledge, Skills, & Abilities:

  • Access to a personal vehicle.
  • Knowledge of Milwaukee County TCM, CCS and/or CSP/ACT Case Management.
  • Experience with individuals with dual diagnoses is preferred.
  • Experience with motivational interviewing is preferred.
  • Knowledge of using work related equipment and applications such as cell phone and texting, computers, copy machine, Microsoft Office, and Teams.
  • Effectively communicates and develops good working relationships with co-workers and outside agencies.
  • Must be familiar with Microsoft Word, Excel, Teams and how to use a cell phone, computer, and fax machine.
  • Reliability in reporting to work regularly and on time.
  • Must possess good communication, listening, writing and organizational skills.
  • Effectively manages one's own time, priorities, and resources to achieve goals.
  • Build and maintain client satisfaction with the services offered by the organization.
  • Diligently attends to details and pursues quality in accomplishing tasks.

  • An understanding of mental health, substance use, and co-occurring needs and the ability to clearly communicate these to others.

  • The ability to maintain and promote positive working relationships and work openly and effectively with one's direct supervisor, colleagues, and outside agencies.
  • Knowledge of Milwaukee County TCM, CCS and/or CSP/ACT Case Management is preferred.

Physical Requirements, Visual Acuity, and Work Conditions:

Physical Requirements: Onoccasion must be able to push/pull/lift/move objects weighing at least 20 pounds.Must have the ability to climb at least two flights of stairs. Work is performedlargely in the community but may at times include an office environment or other various settings.

Visual Acuity: Employee is required to have visual acuity to determine the accuracy, neatness, and thoroughness of the work assigned or to make general observations of facilities or structures

Working Conditions: This is a community-based position. Staff are expected to utilize their personal vehicles to transport clients and meet with clients in their homes. This position may work in a variety of indoor and outdoor work environments which may include various outdoor weather conditions, loud noises, fluctuating temperatures and the occasional use of stairs.The hours of work will generally be during regular business hours and average at least 40 hours a week. This position requires participation in reoccurring 'on-call' duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekends.
