Bright Path Social Work is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our remote team in Saint Petersburg, Florida. This full-time role is designed for individuals with a strong commitment to enhancing the lives of seniors through empathetic support and tailored care. The ideal candidate will possess a master’s degree in social work and a valid state license, along with at least two years of relevant experience in geriatric social work.

This position allows for a flexible work schedule, primarily from the comfort of your home, with occasional in-person visits to clients as needed. Our team is equipped to provide remote support while ensuring seamless communication with clients and healthcare professionals. Although the standard working hours are Monday to Friday from 8:00 AM to 4:30 PM, the role may require flexibility for evening or weekend hours to meet client needs and crises as they arise.

• Conduct comprehensive assessments of the psychosocial needs of seniors, taking into account their emotional, mental, and social circumstances.
• Develop personalized care plans in collaboration with clients and their families, ensuring that individual preferences and goals are prioritized.
• Provide counseling services to seniors, offering emotional support during transitions related to aging, health issues, or family dynamics.
• Act as a client advocate by connecting them to community resources and services, ensuring comprehensive support for various aspects of their lives.
• Collaborate with a multidisciplinary team, including nurses and healthcare providers, to enhance client care and communicate any changes in the client’s status.
• Visit clients regularly to monitor their living conditions, well-being, and to address immediate concerns related to their health or safety.
• Facilitate family meetings to foster clear communication, resolve disputes, and encourage a collaborative approach to client advocacy.
• Educate seniors and their families on available community resources, support groups, and programs, promoting social engagement and overall wellness.
• Maintain detailed and accurate records of client interactions, assessments, and progress notes following best practices in documentation and compliance.
• Stay updated on relevant social work practices and regulations to ensure all services are delivered ethically and effectively.

This role primarily focuses on seniors who may be experiencing complex life transitions or health challenges. As a Licensed Master Social Worker at Bright Path, you will navigate the intricacies of aging and provide expert guidance to seniors and their families, ensuring support during significant life changes, such as hospitalization, loss of independence, or end-of-life planning.

• A Master’s degree in Social Work from an accredited institution.
• Active and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of professional experience in a social work environment, with a focus on geriatric populations preferred.
• Deep understanding of social work principles, including aging-related issues, mental health, and related community resources.
• Strong assessment, counseling, and crisis intervention skills.
• Exceptional empathy, ability to establish rapport, and communicate effectively with seniors and their families.
• Proficient in using electronic health records systems and standard office software applications.

• Experience in a community health clinic or similar setting, providing comprehensive care to diverse populations.
• Familiarity with local community resources and support groups for seniors.
• Prior experience managing a caseload and coordinating care across multidisciplinary teams.
