RN Director of Care Management Acute Care Leadership Experience Required Midwest 

A respected acute care hospital in the Midwest is seeking an RN Director of Care Management to lead a well-established department in a complex inpatient setting. This leader will oversee the development, implementation, and ongoing management of Care Management and Resource Management programs, with responsibility for patient progression, discharge planning, regulatory compliance, and team leadership.

The team itself is on solid footing. What the organization needs now is a leader who brings presence, credibility, and strong operational judgment. The ideal candidate stays connected to the day-to-day realities of the department while still leading from a strategic level. This is someone who can assess opportunities quickly, create alignment across the team, and help strengthen length of stay performance, discharge planning, and overall departmental consistency.

Position Highlights

  • Leads a 20+ FTE Care Management department
  • Supports a daily census of approximately 250 to 300 patients
  • Reports directly to the CFO
  • Oversees Care Management and Resource Management program operations
  • Key areas of focus include length of stay, discharge progression, resource utilization, compliance, and departmental accountability
  • Highly visible leadership role with close partnership across physicians, hospital leadership, and community agencies
  • Opportunity to lead both strategically and operationally in a complex acute care environment

Requirements

  • RN required
  • Bachelor’s degree required or 7+ years of direct experience in lieu of a bachelor’s degree
  • Master’s degree preferred
  • 3+ years of experience in a closely related field required
  • 3+ years of previous leadership experience in an acute care hospital setting
  • BLS required within 7 days of hire or transfer
