General Summary:
Provides professional nursing care to patients within Physician's Practice. Professional nursing practices, requiring specialized knowledge, judgment, and skill are performed in accordance with practice policy, procedure and philosophy
Bayhealth offers the full spectrum of in-hospital and outpatient services for wound care. This includes: Diabetic ulcers, Neuropathic ulcers, Pressure ulcers (bed sores), Burns or trauma wounds, Soft tissue wounds from radiation therapy or injury, Venous and arterial ulcers, venous insufficiency, post-surgical wounds, & other non-healing wounds.
Responsibilities:
1. Demonstrate the ability to provide the following patient care services:a. Perform triage services for patientsb. Prepare injections as required and according to treatment plans designed by the physicianc. Review medication refills with patient via telephone. Ensure that the patient is periodically assessed in the office by the physician to determine therapeutic effect and patient compliance. Adjust patient medications as neededd. Assist with minor medical/surgical procedures in the practice settinge. Demonstrate the ability to recognize abnormal lab results. Required to review lab reports with patients in the absence of the provider.f. Coordinate further testing and evaluation with the physiciang. Identify the need for patient education. Advise patients on their treatment plan; to include Diabetes and other chronic diseasesh. Maintain treatment records/files for each patient Complete patient encounter form at the conclusion of each service performedi. Escort patient to the check-out station at the conclusion of each servicej. Return patient phone calls regarding testing and/or treatment plank. Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice
2. Demonstrate ability to manage the patient treatment area as follows:a. Ensure compliance with Joint Commission regulations and other safety requirements; this includes disposal of used sharps and contaminated materials, as well as, safe storage of medicationsb. Re-order supplies as required and within budgetary limitationsc. Submit requests for capital equipment during the annual budget development processd. Maintain work station cleanliness e. Maintain instruments and equipment; ensure all are sterilized, arranged, and ready for use
3. Demonstrate the ability to reflect knowledge of current professional practice standards, laws and regulations:a. Evaluate his/her own nursing practice in relation to professional practice standards, relevant statutes and regulationsb. Engage in an annual self-assessment, identifying areas of strength and areas requiring professional developmentc. Seek and reflect on constructive feedback from other members of the healthcare teamd. Takes action to achieve performance goals
4. Demonstrate the ability to acquire and maintain current knowledge and competency in nursing care:a. Participate in ongoing educational activities to acquire and validate knowledge i.e. CPR, CLIA training programsb. Seek experiences that reflect current clinical competencies and new clinical informationc. Maintain professional affiliationsd. Attend regularly scheduled staff meeting as required by the practicee. Perform all other tasks and projects assigned by the Practice Manager or Physicianf. May be required to act as back-up for other members of the practice staff.
5. Demonstrate the ability to interact with and contribute to the professional development of peers and other healthcare providers:a. Share knowledge, skills, and experience with colleagues and othersb. Provide colleagues and peers with constructive feedback regarding their practicec. Contribute to a learning environment that is conducive to healthcare educationd. Contribute to a team environment by working with others in a way that promotes and encourages each person's contribution
6. Becomes proficient and cross-trained in the front desk/ administrative area to maintain the front desk as needed, including answering phones, filing, and other duties to keep the flow of the office moving
7. All other duties as assigned within the scope and range of job responsibilities
Required Education, Credential(s) and Experience:
- Education: High School Diploma or GED
Certificate Program; Licensed Practical Nurse (LPN) or equivalent credit hours in an RN program. - Credential(s): Licensed Practical Nurse (LPN)
Basic Life Support (BLS) - Experience: Wound care, physician practice experience
Preferred Education, Credential(s) and Experience:
- Education: None
- Credential(s): Advanced Cardiovascular Life Support (ACLS)
Certification related to the position, if applicable. - Experience: 2 or more years clinical nursing experience in a physician's practice strongly preferred
