Job description

The Medical Records and Patient Services Coordinator is a cross-trained position responsible for performing medical records functions and coordinating patient referrals to support timely,accurate, and seamless patient care.
The position is located at the AvitaCare Atlanta Clinic (2140 Peachtree Road).
Enjoy a work schedule with no weekends, holidays, or late evenings! Standard work hours are Monday through Friday, 8:30 AM to 4:30 PM.
About the Role:

  • Manages patient health information by processing release-of-information requests, maintaining complete and accurate medical records, coordinating
    specialty referrals, verifying insurance and demographic information, and communicating with patients and healthcare providers
  • Performs medical record audits and ensures records are complete prior to filing, refiling, or closing related tasks
  • Responsible for handling more complex records requests with the support of legal/compliance teams regarding court requested records
  • Sort, file, scan, upload, and collate medical records and information, including progress notes, treatment plans, nursing/clinical notes, discharge summaries, lab and imaging reports, hospital records, referral information, test results, and other patient documentation
  • Creates and maintains medical record files and ensures records are complete, accurate, timely, and filed appropriately in the patient chart
  • Process requests for medical records from outside agencies, third-party sponsors, healthcare providers, and other authorized requestors
  • Maintains release-of-information documentation, completed ROIs, logs, statistics, and other required records in accordance with legal, compliance, and organizational requirements
  • Coordinates and process all patient referrals for specialty services, hospital services, outpatient services, and other patient specialty health services
  • Monitors outstanding referral reports, missing consultation notes and pending documentation
  • Assists designated staff in locating records, documents, and referral-related information
  • Obtains and verifies registration data, demographic information, insurance coverage, eligibility, and required authorizations with a high degree of accuracy
  • Completes referral forms, requisitions, authorizations, and related administrative documentation as needed
  • Schedules referral appointments and contact patients in a timely manner regarding referral requirements and scheduling needs
  • Acts as a liaison between Avita Care Atlanta, internal providers, external physicians, specialty offices, hospitals, and other healthcare partners while demonstrating strong customer service
  • All duties are performed in compliance with HIPAA, Avita Care Atlanta policies, and applicable regulatory requirements
  • Additional responsibilities assigned as needed
About You:
  • High school diploma or equivalent required
  • Associate degree or bachelor's degree in a healthcare-related field preferred
  • Certification in health records preferred
  • One year experience in medical referrals, medical records, or related medical patient support services in an ambulatory setting preferred
  • Knowledge of medical records processes, release-of-information requirements, confidentiality standards, and HIPAA guidelines required
  • Experience with quantitative medical record reviews preferred
  • Maintains a high level of patient confidentiality
  • Demonstrates attention to detail and the ability to multi-task
  • Strong customer service skills and the ability to serve as a professional liaison between patients, providers, and outside organizations
  • Bilingual in Spanish preferred
  • Proficient in English, with strong verbal and written communication skills
  • Ability to use computer systems, electronic health records, Microsoft Office, and general office equipment
  • Experience with eClinical Works preferred
