Kaiser Case Manager (LVN)
Vasona Creek Healthcare Center is seeking a highly organized, compassionate, and proactive Licensed Vocational Nurse (LVN) Case Manager to join our team. This key role is responsible for coordinating patient care, managing utilization reviews and authorizations, and facilitating seamless transitions of care for our Kaiser patients.
The ideal candidate will have strong clinical knowledge, excellent communication skills, and experience working with managed care plans in a skilled nursing or long-term care setting.
Position Highlights

Key Responsibilities
  • Oversee and coordinate the discharge planning process from admission through transition, with a primary focus on Kaiser patients and their care partners.
  • Conduct utilization reviews and collaborate with Kaiser, managed care plans, insurance providers, and other healthcare partners to obtain necessary authorizations.
  • Serve as the primary liaison among patients, families, physicians, Kaiser representatives, and interdisciplinary care teams.
  • Monitor resource utilization to promote efficient, cost-effective, clinically appropriate, and compliant care delivery.
  • Develop and maintain accurate, timely documentation, including care plans, case management notes, utilization reviews, and regulatory records.
  • Identify barriers to discharge and work proactively with the interdisciplinary team to facilitate timely and safe transitions of care.
  • Communicate changes in patient status, treatment needs, and discharge plans to appropriate healthcare partners.

Qualifications
Required
  • Current, unrestricted California Licensed Vocational Nurse (LVN) license.
  • Minimum of 2 years of nursing experience.
  • Strong clinical assessment, communication, organization, and problem-solving skills.
  • Ability to effectively collaborate with patients, families, physicians, insurance representatives, and interdisciplinary teams.

Preferred Experience
  • Case management or utilization review experience.
  • Experience with Kaiser Permanente patients or managed care plans.
  • Charge Nurse or Admissions experience.
  • Previous experience in skilled nursing, post-acute care, or long-term care.
  • Strong knowledge of Medicare, Medi-Cal/Medicaid, managed care, and insurance authorization requirements.
  • Demonstrated ability to advocate for patients while balancing quality outcomes, regulatory compliance, and appropriate resource utilization.
