Licensed Practical Nurse (LPN)

Full-Time | Part-Time | All Shifts Available

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide direct nursing care and support residents’ daily clinical needs. This role plays a key part in maintaining quality care, safety, and dignity for our residents while working closely with the nursing and interdisciplinary teams.

Responsibilities

• Provide direct nursing care in accordance with care plans and physician orders
• Administer medications and treatments as ordered
• Monitor residents for changes in condition and report concerns promptly
• Accurately document care, treatments, and observations
• Assist with admissions, transfers, and discharges
• Supervise and support CNAs/GNAs to ensure quality resident care
• Follow infection control practices, safety standards, and facility policies
• Communicate effectively with residents, families, and care team members

Qualifications

• Current, active LPN license in the state
• Ability to work collaboratively in a team-based environment
