We are seeking an experienced Acute Care RN Case Manager to coordinate patient care, discharge planning, and utilization management in a fast-paced acute care hospital. The Case Manager will collaborate with physicians, nurses, social workers, and interdisciplinary teams to ensure safe, timely, and cost-effective patient care while facilitating smooth transitions across the continuum of care.

Key Responsibilities

  • Perform comprehensive case management and discharge planning for acute care patients.
  • Coordinate care with physicians, nursing staff, and interdisciplinary teams.
  • Assess patient needs and develop individualized discharge plans.
  • Facilitate referrals for post-acute services, DME, home health, SNF, rehab, and community resources.
  • Monitor length of stay, resource utilization, and barriers to discharge.
  • Ensure compliance with hospital policies, CMS regulations, and payer requirements.
  • Maintain accurate and timely documentation in the EMR.

Minimum Requirements

  • Active California RN License (Pending license accepted but must be active by start date)
  • Minimum 2 years of Acute Care Hospital Case Management/Discharge Planning experience
  • Hospital Case Management experience is required (Behavioral Health experience alone does not qualify)
  • Experienced travel nurse required (First-time travelers will not be considered)

Schedule & Additional Details

  • Start Date: ASAP

  • Floating: Not Required
  • Weekend Rotation: Every other weekend (based on department needs)
  • Patient Ratios: Days 1:25 | Weekends 1:45
