Job description

UNM Medical Group, Inc. is hiring for a Manager of Case Management Services to join our Truman Health Services team. This opportunity is a full-time day shift opening located in Albuquerque, New Mexico.

Minimum $77,940 - Midpoint $99,332*

*Salary is determined based on years of total relevant experience.

*Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.

Summary

Provides direct oversight of the Case Management team on day-to-day operations. Manages and maintains a high-quality Case Management program designed to move patients safely and efficiently from initial intake to assessment to coordinated care planning and ongoing patient management. Develops and oversees the standardization of practices for the Case Management team which promotes high quality care by interfacing with interdisciplinary team members.

Minimum Job Requirements of a Manager, Case Management Services:

Associates and/or Bachelor's degree from an accredited institution and 5 years of experience working with patients populations that are HIV diagnosed, have chronic health illness, and/or a part of an underserved population(s). Education above the minimum requirements can be substituted on a year-for-year basis. Verification of education and licensure (if applicable) will be required if selected for hire.

Required License:

  • Possession of a valid unrestricted NM driver's license.

Duties and Responsibilities of a Manager, Case Management Services:

  • Establish case management standard operating procedures (SOP) to ensure all case management components (intake, assessment, medication adherence, assessment, documentation, referrals, patient assistance programs, etc.) are implemented according to program compliance requirements; ensure all SOPs are well documented, implemented according to protocol and understood by direct reports and external team members.
  • Evaluates effectiveness of the case management program; works with Program Operations Director and Manager, Clinical Quality Programs to identify areas for improvement and conduct quality improvement initiatives to ensure continual quality and compliance is met.
  • Develops documentation templates and enforces appropriate use in electronic health records.
  • Creates and maintains database to inform case management reports and updates to Program Operations Director and Medical Director.
  • Recruits, hires, supervises, trains, and evaluates a team to provide patient support services.
  • Working closely with the Program Operations Director, identifies and solicits local, state, federal, and/or international funding sources for services and programs.
  • Assists with direct patient case management, as needed, when short-staffed or with highly complex cases.
  • Regularly conducts audits of direct reportsโ€™ work to ensure adherence to standard operating procedures, State and Federal grants as well as ensure quality case management.
  • Foster relationships with external community-based organizations to expand current community resource repository to address client needs.
  • Maintains current capacity of case manager panels, ensuring reassignment as needed to ensure proper coverage.
  • Performs miscellaneous job-related duties as assigned.
