Job description

Nurse Practitioner Comprehensive Adult Care

We are seeking a compassionate, mission-driven Nurse Practitioner to provide high-quality, value-based care to older adults with complex medical needs. This role emphasizes outcomes over volume and close collaboration with an interdisciplinary care team.

Responsibilities

  • Perform comprehensive adult and geriatric assessments in clinic, home, senior living, and via telehealth

  • Manage transitions of care to reduce avoidable hospital and emergency department visits

  • Identify care gaps and collaborate with primary care providers to optimize care plans

  • Evaluate and manage non-life-threatening acute conditions

  • Support chronic disease management, preventive care, and advance care planning

Schedule

  • Full-time, MondayFriday (8am5pm)

Qualifications

  • Current Oregon Nurse Practitioner license

  • ANCC or AANP certification

  • 35 years of NP experience (Primary Care, Geriatrics, Internal Medicine, Family Medicine, or Palliative Care preferred)

  • DEA licensure (or ability to obtain) and CPR/BLS certification

  • Spanish bilingual a plus
