Job description

Under the direction of the program manager, provides supervision and direction and training to a team of case managers, and provide suggestions and recommendations to the program manager as to the hiring, advancement, and progressive discipline of case managers and potential case managers on their team. Coordinator also maintains a caseload with the same responsibilities as the case managers.

  • Clinic Hours are Monday through Friday, 8AM to 5PM (Actual hours may vary depending on business need)

EDUCATIONAL REQUIREMENTS

  • Bachelor’s degree from an accredited four-year college or university required
  • Licensed as a social worker (any level), RN, or counselor; or Masters degree from an accredited institution of higher education preferred

QUALIFICATION

  • Five or more years case management experience
  • Evidence of superior case management skills in terms of ability to engage clients, treatment planning, networking, service delivery, administrative protocols, and professional conduct
  • Maintains auto insurance as acceptable by minimum standards; in addition, names Mojave as an additional insured/interested party and maintains “business use” on policy during course of employment
  • Automobile in good working condition, in order to transport clients if needed
  • Current and valid driver’s license
  • In accordance with the conditional offer of employment, individual must pass drug screen and background investigation

PHYSICAL REQUIREMENTS

  • May include standing, sitting, and/or walking for extended periods
  • May include performing repetitive tasks
  • May include working on a special schedule (i.e. evenings and weekends with clients)
  • May include working with challenging patients and clients
