RN Care Coordinators work with physicians, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care experiences and services, to improve population health, reduce cost, and to achieve collaborative practices that exemplify Main Line Health System's (MLH) commitment to patient centered care, community engagement, and population health. RN Care Coordinators lead and are accountable for providing exemplary cross continuum care coordination and care transitions, and are responsible for partnering with patients, families, physicians and colleagues to develop, implement and monitor a comprehensive plan of care; inclusive of health promotion, disease prevention, and disease management activities.

Position: Care Coordinator RN 
Shift: Monday-Friday; day shift plus weekend and holiday coverage
Experience: 
3 years of recent clinical experience required, leadership experience preferred. If ADN, 10+ years of experience required.
Education: 
1. Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
2. Master's degree preferred
Licensures/Certifications: 
Active PA RN license required
